ATHLETIC CLEARANCE APPLICATION

An athletic clearance is mandatory for any student wanting to try out for a sport. Once completed, the student will be cleared for that school year only. Students are not allowed to participate (including tryouts or practice) unless they have athletic clearance. packet. Please return completed physical to the Athletic office and submit all electronic forms.

Step 1: Under the Download/Print tab access the Physical/Medical History form. Print off the blank copy and take it with you to your child’s physical appointment. After the physical is completed please have your student turn it into the Athletic Office.

Step 2: Complete all Electronic Participation Forms. To access the online forms hold your curser over the “Electronic Participation Forms” tab. You will get a drop down list of the forms. Click on the form name and fill out the information requested. To sign the document, click inside the signature box and hold your mouse over the box. This will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click the refresh icon next to the signature box.

-Click on the hyperlinks to read and review the following documents: Parent Letter, Athletic Contract, CIF Ethics in Sports, Concussion Acknowledgement, Steroid Prohibition Agreement
-After you have read each form, place a check in the box to acknowledge that you have read and understand the information in the documents and electronically sign the form.
